1. the Subject of the Order is Delivery to the Provincial Hospital in Bielsko-biala at Al. Armii Krajowej 101, Implants and Operating Materials for Neurosurgical Procedures, According to the Quantitative and Generic List Specified in Annex No. 2 to the Terms of Reference, Hereinafter Referred to as "swz", Including By Depositing the Ordering Party for Part 12, Item 5 and Part 13, Meeting the Quality Parameters Specified in Annex No. 2 to the Swz, Under the Conditions Specified in the Draft Provisions of the Contract Constituting for Parts 1-11 - Annex No. 4a to the Swz, for Parts 12-13 - Annex No. 4b to the Swz. 2. the Offered Goods Must Be New and Unused. 3. Delivery of the Subject of the Order, Including Transport and Unloading, Will Take Place at the Expense and Risk of the Contractor. 4. Applies to Part 13 - for the Duration of the Contract, the Contractor is Obliged, as Part of the Price of the Subject of the Contract, to Provide and Deposit at the Ordering Partys Office Two Sets of Tools, Consisting Of: a Phillips Screwdriver, Pliers for Cutting Tiles, Pliers for Bending Tiles, Tweezers, a System for Storing a Magazine for 100 Screws, Magazines for 6 Screws, Space for Plates and a Screwdriver Blade, a Drawer for Meshes, for the Implantation of Implants Specified in the Part 13, Along with a Container for Sterilization and Sterile Storage of Instruments.
